Program Overview - Supportive Housing Programs

Bracondale House provides supportive housing for seniors and families, including individuals who may have experienced homelessness, mental health challenges and/or substance use. The program provides on-site supports and services including assistance with activities of daily living, social and recreational activities, volunteer opportunities, and follow-up support to promote stability, independence, and well-being.

Position Summary

The Manager, Resident Care Services provides day-to-day leadership for Bracondale's supportive housing operations and resident support services, while ensuring strong administrative coordination across the site. This role oversees supervisors/team leads, frontline staff, volunteers, and students, ensuring services are delivered safely, consistently, and in alignment with organizational policies, funder expectations, and program outcomes.

The Manager, Resident Care Services plays a key role in service quality, staff scheduling and performance, care planning and case coordination, safety and emergency response, tenancy supports, documentation/reporting, and continuous improvement, while building a positive, accountable, and resident-centered team culture. This role participates in an on-call rotation for Supportive Housing Programs. This is a current vacancy.

What You Will Do

Program Leadership and Daily Operations

  • Oversee day-to-day operations for supportive and/or transitional housing programs, services, and initiatives.
  • Lead, coach, and support Supervisors, Team Leads, and frontline staff to achieve program outcomes.
  • Provide regular team and one-to-one meetings to support team cohesion, service consistency, and effective crisis response.
  • Implement, maintain, and improve operational protocols, policies, and procedures in consultation with the Director/ED.
  • Promote program awareness and represent Bracondale in relevant internal and external committees, networks, and partnerships.
  • Support the Director/ED in strengthening internal and external partnerships and collaborative activities.

Staffing, Supervision, and People Management

  • Provide orientation, training, supervision, and annual performance appraisals for staff.
  • Support recruitment and hiring, and assist with personnel functions in consultation with the Director/ED.
  • Coordinate staffing schedules and ensure appropriate coverage for services (including evenings/weekends as required).
  • Identify training needs and support staff development planning with the Director/ED.
  • Ensure the effective utilization, training, and support of volunteers and students.

Resident Support, Case Management, and Care Planning

  • Support staff to complete assessments and develop, monitor, and update individualized Care Plans.
  • Coordinate support services including personal care, meal supports, housekeeping, and companionship, and monitor service quality.
  • Maintain a resident-centred approach that promotes independence, dignity, safety, and emotional well-being.
  • Use best-practice tools and documentation standards (e.g., InterRAI/CHA and CAPs where applicable) and ensure timely, accurate record-keeping.
  • Ensure staff and department are adhering to privacy legislation (e.g., PHIPA) and documentation protocols.

Health System and Community Coordination

  • Liaise with external healthcare and community partners (e.g., Ontario Health / Home and Community Care Support Services, hospitals, physicians, pharmacies, community providers) to coordinate supports and care transitions.
  • Participate in care conferences and coordinate referrals to health professionals and housing/LTC providers as needed.
  • Monitor resident status trends and support staff in escalating concerns appropriately.

Safety, Emergency Response, Infection Prevention, and Risk

  • Implement and oversee safety protocols, emergency preparedness, and incident response procedures.
  • Respond to urgent situations (including emergency calls), coordinate with emergency responders, and ensure incident documentation and follow-up.
  • Ensure team compliance with infection prevention and control measures, PPE use, and relevant organizational requirements.
  • Report risk, safety, security, liability, and building-related concerns in a timely manner to the Director/ED, and assist in implementing solutions to improve safety and wellness.

Tenancy Support and Housing Stability

  • Develop and maintain procedures for tenant selection and internal transfers.
  • Work with Housing Staff to address tenancy and safety issues (e.g., hoarding, fire safety, trespassing, unsafe behaviours) and appropriate alternatives to eviction and support strong housing stabilization practices.
  • Anticipate turnover and work proactively to support low vacancy rates.

Administration, Reporting, and Financial Oversight

  • Prepare and monitor program budgets and financial information in alignment with administrative policies and procedures.
  • Develop and maintain data tracking systems; liaise with funders and complete quarterly and other required submissions and reports.
  • Prepare quarterly and ad-hoc statistical and program reports, including outcomes, risks, and recommendations.
  • Participate in unit/department leadership meetings and support coordinated, integrated service delivery across Bracondale.

Additional Duties

  • Carry out other duties as assigned by the Director/ED to support organizational priorities and effective operations.

What You Bring to the Team

  • Bachelor's degree in healthcare or relevant field (ex. Nursing, Social Work) or equivalent combination of education and experience is required.
  • Minimum 5 years' experience in supervisory/managerial role, while overseeing/developing/ implementing programs, working with seniors and vulnerably housed individuals is required.
  • Experience in Supportive Housing and knowledge of the Residential Tenancies Act (RTA), occupancy agreements, and property management practices is required.
  • Clear and Updated Vulnerable Sector Screening and First Aid Certification are required.
  • Sound judgment, flexibility, creativity, and sensitivity to changing needs and crisis situations.
  • Demonstrated ability to lead teams in complex, fast-paced environments with professionalism and empathy.
  • Strong computer proficiency and demonstrated experience and comfort utilizing online client databases (such as CIMS, PointClickCare, Alayacare) and electronic documentation.
  • Strong understanding of supportive housing milieus & long-term care advocacy/services, senior care and aging support, Toronto healthcare services and resources, eviction prevention, mental health & substance use, and the ability to translate this into effective service delivery.
  • Proven ability to develop program initiatives and partnerships that improve resident outcomes.
  • Excellent interpersonal and communication skills across staff teams, tenants, and external partners.
  • Strong facilitation, conflict resolution, negotiation, and crisis prevention/intervention skills.
  • Demonstrated financial and budget management capacity.
  • Strong writing and editing skills and comfort with data tracking/reporting systems.
  • Knowledge of privacy requirements (e.g., PHIPA) and best practices in confidential information handling.
